FSM to unknown state after SIGNED status. Broken account?

A specific account of mine cannot open new channels, even after signing successfully the create channel transaction.

I verified the SDK showing the FSM transitioning thru accepted/connected/signed. After this, it goes into the Unknown FSM state exception.
